  • Direct Care positions

ELM Homes is seeking self-motivated, compassionate, fun-loving employees to provide services to individuals with developmental and/or physical disabilities; assist with activities of daily living, community integration, and encouraging independence. Must be 18 or older, drive company vehicles, and background checks are required. All positions include every other weekend. Full-time and part-time hours are available. AA/EOE

  • Health Coordinator (HC) Positions: 

Responsibilities include, but are not limited to, accompanying individuals on medical appointments, maintaining medical files, and monitoring compliance of health care plans. Position includes on-call time. Work flexible hours, while earning paid vacation. Must be 18 or older. Must have further education or experience in health care field. Wage DOE. Must be able to drive company vehicles and background checks are required. AA/EOE

  • Supervisory Positions:

Residential Program Manager (RPM) Position:  This is a house manager position. Previous supervisory experience is preferred. Responsibilties include, but are not limited to, staff supervision, assisting individuals with activities of daily living, financial supervision, community integration and encouraging independence. Must be at least 18 years of age, willing to work direct care shifts, take on call, have good communication/comprehensive speaking and writing skills of the English language, have a valid driver's license with no restrictions, have a telephone or some means of communication when out of the facility, and background checks are required. Wage DOE. AA/EOE

Designated Coordinator (DC) Position:  This is a multi-site manager position. Previous supervisory experience is required along with the following:

MUST HAVE ONE OF THESE:

(1) a baccalaureate degree in a field related to human services, AND one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older;

(2) an associate degree in a field related to human services, and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older;

(3)A diploma in a field related to human services from an accredited postsecondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older;

OR MUST HAVE BOTH OF THESE:

(4) a minimum of 50 hours of education and training related to human services and disabilities; and

(5) four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications identified in clauses (1) to (3).

Responsibilities include, but are not limited to, staff supervision, goal writing and follow-up, lead team meetings, communicate with county case managers or other team members.

Must be at least 18 years of age, willing to work direct care shifts, take on call, have good communication/comprehensive speaking and writing skills of the English language, have a valid driver's license with no restrictions, have a telephone or some means of communication when out of the facility, and background checks are required.
